Performance Evaluation Process

As a participating co-op employer, we ask that you fill out a final performance evaluation for your co-op student. Your co-op student will present the form to you near the end of the co-op experience. The desired goal is that the direct supervisor will fill out the form and discuss the evaluation directly with the student. We also encourage the supervisor and or mentors to give feedback to the co-op student on his or her performance throughout the term as time permits.
